Baby Dedication

To schedule a baby dedication, it is necessary to fill out the form with the baby's and parents' details.

Please inform the parent's full name (mother and father)

Please inform Baby's Gender

Please provide at least two date options for the Baby Dedication. Kindly note that the chosen date will need to be approved to ensure it does not conflict with any church events.
